HIPAA Authorization to Use and Disclose Protected Health Information Forms
The Privacy Rule (at 45 CFR 164.512), states that the Authorization Forms should be approved by the Covered Entity's Privacy Officer.
If requested, New England IRB will review, and if appropriate, approve Authorization Forms. However, as the Privacy Rule affords the IRB no authority to approve the Authorization Forms, New England IRB will review the Forms in accordance with IRB regulations (21CFR 50 and 56).
